To claim a payout of basic deposit compensation, a client does not have to fill out any form. They only need to identify themselves at a payout point with a valid ID. If a client is a legal entity they must present a document entitling them to collect the deposit. You can find the list of required documents and IDs here. In case a client cannot collect the compensation in person, they can:
- authorise someone to collect the compensation by means of a power of attorney. For convenience, we have provided a power of attorney form – power of attorney.
- or request payout of the compensation via bank transfer by correspondence: A written instruction with an officially certified signature of the entitled person has to be sent to the payout bank address: Komerční banka, a.s., CKB department 2192, Nám. Junkových 2772/1, Praha 5 – Stodůlky, postcode 155 00. A sample “Compensation Transfer Request for natural person” as well as for “Compensation Transfer Request for legal person” are available on the Documents required to obtain compensation page. If the signature is certified outside the Czech Republic, the certification must be performed by an embassy of the Czech Republic.
If you are entitled to increased compensation, you must fill out an application form and enclose the relevant documents.
If a client disagrees with the compensation amount paid, he/she can initiate the complaint process. Here you can find the complaint form, which upon completion you should send to the Guarantee System.
